HER2-positive gastric and gastroesophageal junction (GEJ) adenocarcinoma — defined as IHC 3+ or IHC 2+/ISH+ — affects 15–20% of patients with advanced disease. The KEYNOTE-811 Phase 3 trial (n=698) demonstrated that adding PD-1 checkpoint inhibition to trastuzumab plus chemotherapy significantly improved overall survival to 20.0 months vs. 16.8 months with placebo (HR 0.81; p=0.0093), representing the first OS benefit in HER2-positive gastric cancer beyond trastuzumab alone. The benefit was most pronounced in patients with combined positive score (CPS) ≥1.
